Serum Lactate Mortality Index (post-CPB): Explanation and Clinical Context The Serum Lactate Mortality Index (SLMI) is a prognostic tool used to estimate the risk of postoperative mortality in patients after cardiopulmonary bypass (CPB). It is calculated by multiplying peak serum lactate levels (mmol/L) by the time elapsed since CPB (hours). Elevated lactate levels reflect impaired tissue perfusion and oxygen delivery, which may indicate hemodynamic compromise or low cardiac output postoperatively.

The SLMI integrates both the magnitude and duration of hyperlactatemia, improving the predictive accuracy for early postoperative outcomes. Low SLMI values generally suggest minimal postoperative risk, while higher values indicate increased likelihood of mortality and need for close monitoring or interventions.
