SPRINT Trial Risk Stratification: Explanation and Clinical Context The SPRINT Trial (Systolic Blood Pressure Intervention Trial) risk stratification is a clinical tool designed to estimate the expected benefit and risk of intensive (<120 mmHg) versus standard (<140 mmHg) systolic blood pressure control in patients with hypertension. It incorporates age, chronic kidney disease (CKD) status, and estimated 10-year ASCVD risk to classify patients into low, moderate, or high-risk categories. High-risk patients, including those aged >75 years, with CKD, or with ASCVD risk >15%, derive the greatest absolute benefit from intensive blood pressure lowering but require close monitoring for adverse events such as hypotension, syncope, or acute kidney injury.

Moderate-risk patients may benefit from intensive control but with a lower absolute risk reduction. Low-risk patients have limited incremental benefit from intensive therapy. This stratification informs individualized blood pressure targets and guides shared decision-making regarding intensity of antihypertensive therapy.
