The calculation integrates age-related risk, resting heart rate, cumulative exposure (in years), and the physiological impact of the specific substance used. Chronic nicotine exposure increases sympathetic tone, resting heart rate, and arterial stiffness, leading to higher risks of arrhythmia and endothelial dysfunction.

Cannabis use is associated with transient tachycardia and coronary vasospasm, while stimulants (e.g., cocaine, amphetamines) markedly raise the risk of sudden cardiac death, especially during exertion. Anabolic steroid use induces concentric left ventricular hypertrophy, diastolic dysfunction, and can precipitate myocardial fibrosis and arrhythmias. For competitive athletes, the combination of high catecholamine surge during training and chronic exposure to these substances significantly multiplies the probability of exercise-induced cardiac events.
