The original score was derived from data of over 10 000 patients in the STS/ACC TVT Registry (2015‑2017) and externally validated in a separate cohort. Key predictors include patient comorbidities (such as diabetes mellitus, atrial fibrillation), functional status (home‑oxygen use), procedural complications (major vascular complications, post‑procedure regurgitation), laboratory data (elevated creatinine), and hospital‑stay metrics (length of stay > 5 days).

Stratification into low, moderate and high risk groups allowed clinicians to target post‑discharge interventions and optimise discharge planning. For example, in the derivation cohort the low‑risk group had ~5.8% readmission vs~14.6% in the high‑risk group.
