Ferritin–Transferrin Saturation Ratio (Iron Deficiency in Heart Failure): Explanation and Clinical Context The Ferritin–Transferrin Saturation (TSAT) ratio is a widely used laboratory tool to assess iron status in patients with heart failure. Ferritin reflects iron storage, whereas TSAT reflects circulating iron availability. Iron deficiency is highly prevalent in heart failure and is associated with impaired functional capacity, higher risk of hospitalization, and worse outcomes.

Absolute iron deficiency is defined as ferritin <100 ng/mL, and functional iron deficiency is characterized by ferritin 100–300 ng/mL with TSAT <20%. This calculator estimates the Ferritin/TSAT ratio and provides a clinically relevant interpretation to guide further iron supplementation strategies in heart failure patients.
