ST2 Biomarker Risk Tier: Explanation and Clinical Context The soluble ST2 (sST2) is a member of the interleukin-1 receptor family and acts as a biomarker of myocardial stress and fibrosis. It is a strong prognostic marker in patients with heart failure and acute coronary syndromes, independently predicting mortality and heart failure readmission. Risk tiers are generally classified based on sST2 concentration: - <35 ng/mL: Low Risk - 35–70 ng/mL: Intermediate Risk - >70 ng/mL: High Risk These thresholds are derived from large-scale studies such as the PRIDE and PROTECT cohorts, which demonstrated that higher sST2 levels correlate with adverse cardiovascular outcomes.

Clinically, sST2 measurement can guide risk stratification, optimize therapy intensity, and improve patient management.
