The model identifies key baseline predictors of the composite of all‐cause death or hospitalization, and of all‐cause death alone. Among the strongest predictors were baseline exercise duration on cardiopulmonary exercise testing, KCCQ symptom stability score, elevated BUN, and male sex.

Clinically, this tool supports risk stratification by identifying higher‐risk patients who may benefit from intensification of therapy (e.g., advanced device therapy, closer monitoring, structured exercise training) and lower‐risk patients for whom routine interventions may be sufficient.
