Jelliffe renal function equation Explanation and clinical context The Jelliffe equation estimates creatinine clearance in adults from age 20 to 80 years by combining an age dependent production term with measured serum creatinine and body surface area. The equation adjusts for sex by applying a ten percent reduction for female sex to reflect lower creatinine production. In this implementation serum creatinine may be entered in either mg per dL or micromol per L.

When micromol per L is used the value is converted to mg per dL by multiplying by 0.0113. The calculated value represents estimated creatinine clearance expressed in milliliters per minute and is intended to support drug dosing and renal function assessment in cases where steady state serum creatinine is present or when an estimate is needed for clinical decision making. Clinical interpretation requires integration with clinical context.

Estimated creatinine clearance values should be compared with prior measurements and interpreted together with urine output hemodynamic status and trends in serum creatinine. The Jelliffe equation may be useful when an estimate that accounts for age body size and sex is preferred. Limitations include reduced accuracy in severe obesity in patients with rapidly changing serum creatinine and in populations that differ from those used to derive the original formula.
