Monocyte–Lymphocyte Ratio (MLR): Explanation and Clinical Context The Monocyte–Lymphocyte Ratio (MLR) is a simple hematological index derived from routine complete blood counts by dividing the absolute monocyte count by the absolute lymphocyte count. It has emerged as a prognostic marker in various cardiovascular and inflammatory conditions. Elevated MLR has been associated with higher systemic inflammation, increased risk of adverse cardiovascular events, and poorer outcomes in acute coronary syndromes.

Clinical interpretation requires context: normal values typically range from 0.2 to 0.4, but thresholds for risk stratification may vary depending on the studied population and disease condition. MLR is easy to calculate, cost-effective, and can complement other inflammatory markers such as NLR (Neutrophil–Lymphocyte Ratio) and CRP (C-reactive protein).
