NT-proBNP/BNP Ratio: Explanation and Clinical Context The NT-proBNP/BNP ratio is a prognostic biomarker used in heart failure and cardiovascular risk assessment. NT-proBNP, the inactive N-terminal fragment of proBNP, and BNP, the biologically active peptide, reflect ventricular wall stress and myocardial dysfunction. Differences in clearance mechanisms and half-lives may cause discrepancies in absolute concentrations; therefore, the ratio can provide refined prognostic insights.

A lower ratio (<1.5) generally indicates lower risk, intermediate ratios (1.5–3.0) suggest moderate risk, and higher ratios (>3.0) are associated with worse prognosis, including increased risk of hospitalization and mortality. Clinical use of the NT-proBNP/BNP ratio should complement other clinical assessments, including echocardiographic parameters, renal function, and patient comorbidities.
