Simplified Acute Physiology Score II (SAPS II): Explanation and Clinical Context SAPS II is a validated intensive care unit (ICU) mortality prediction model designed to estimate the probability of hospital mortality based on physiological variables measured during the first 24 hours of ICU admission. It incorporates 17 parameters: 12 physiological variables, age, type of admission, and three underlying disease variables (AIDS, metastatic cancer, hematologic malignancy). Each variable contributes a specific weighted score derived from multivariate analysis, resulting in a total SAPS II score ranging from 0 to 163.

The mortality risk is then computed using a logistic regression formula from the original derivation cohort. A higher SAPS II score indicates a more severe physiological disturbance and an increased risk of death. It is used worldwide for benchmarking ICU performance, stratifying clinical severity, and guiding discussions about prognosis.
