TAPSE/PASP Ratio: Explanation and Clinical Context The TAPSE/PASP ratio is a non-invasive echocardiographic index used to assess the coupling between right ventricular (RV) function and pulmonary arterial load. TAPSE (Tricuspid Annular Plane Systolic Excursion) measures longitudinal RV systolic function, while PASP (Pulmonary Artery Systolic Pressure) reflects the afterload imposed on the RV. This ratio provides insight into right ventriculo–arterial coupling: higher values indicate preserved RV function relative to pulmonary pressure, while lower values suggest impaired coupling and higher risk of adverse outcomes.

Cut-off values proposed by Guazzi et al., 2013 indicate that a ratio >0.31 mm/mmHg is considered normal, 0.23–0.31 mm/mmHg indicates mild-to-moderate uncoupling, and <0.23 mm/mmHg signifies severe uncoupling with poor prognosis. Clinically, TAPSE/PASP ratio is valuable for risk stratification in patients with pulmonary hypertension, heart failure, and other conditions affecting right ventricular function. It complements other echocardiographic parameters and biomarkers to guide therapy and monitor disease progression.
