Pediatric Crohn Disease Activity Index PCDAI Explanation and Clinical Context The Pediatric Crohn Disease Activity Index is a validated composite instrument used to quantify disease activity in children with Crohn disease. It combines history items including abdominal pain stool pattern and general well being with physical examination findings growth and perianal assessment and laboratory markers including hematocrit erythrocyte sedimentation rate and serum albumin. Each item is scored on a three point scale except for hematocrit and erythrocyte sedimentation rate which use smaller point increments The total score ranges from zero to one hundred with higher scores indicating greater disease activity.

Commonly used interpretation cutoffs are remission less than ten points mild disease ten to twenty seven point five points moderate disease around thirty to thirty seven point five points and severe disease at higher values A change of twelve point five points or greater is often used to define a clinically significant response to therapy Reference Hyams J et al Development and validation of the Pediatric Crohn Disease Activity Index J Pediatr Gastroenterol Nutr 1991 and subsequent validation and appraisal studies including Turner et al Am J Gastroenterol 2010 and publicly available scoring tables.
